Due to COVID-19, all travel requests should be submitted as normal by accessing the online form below. Once we receive and process student’s request, we will email a new electronic I-20 with a digital signature for travel endorsement. We recommend students to print out the new I-20 in color ink when re-entering the US.
STEP 1:
Click on the link for the form that you need, and fill it in electronically. Please be aware that requests will be rejected if we do not receive the supporting documents within five business days.
STEP 2:
If you are using your JWU E-mail:
Providence campus students must submit their requests to the ISA email.
Students must put their J# and the request form title in the subject line of the email. (Example: J01234567 – Travel Signature)
Students from all other campuses, must email their requests to their international advisor.
If you are using your personal (non JWU) E-mail:
All students must use the upload document tab below.
Gramm-Leach-Bliley Act (GLBA)
The Gramm-Leach-Bliley Act (GLBA) is a law that applies to financial institutions and includes privacy and information security provisions designed to protect consumer financial data. This law also applies to how higher education institutions collect, use, and store student sensitive data. The intent of this law is to protect the integrity of student's sensitive information and guard against security threats of unauthorized access that could result in harm or inconvenience to our students.
